How to Register  

Our practice area covers the villages of St Stephen, St Dennis, Roche, Foxhole, Nanpean, Whitemoor, Treviscoe, Indian Queens, Fraddon, Summercourt, Ladock, Probus, Grampound Road, Grampound, Hewas Water, Sticker, Polgooth, Coombe, Lanjeth and the surrounding areas. 

Registration forms are available from the receptionists.  Patients may express a preference as to which doctor they register with however, we do not operate personal lists so you may be offered an appointment with any doctor

Change of Address

If you change name, address or telephone number, please let our receptionists know by calling or writing to us.  If you move outside the practice area you may need to find a doctor in your new area.  There are instructions on what to do when you move to a new area on the back of your NHS Medical Card.

Computerisation

The practice is fully computerised.  As well as patient names and addresses etc, all appointments, prescriptions and clinical details are computerised.  Please make sure you inform us of any changes so that our records are as accurate as possible.  The practice complies with the current Data Protection Act.

Access to Medical Records

The practice is registered and complies with the Data Protection Act 1998.  Any request for access to notes by a patient, patient's representative or outside body will be dealt with in accordance with the Act.  Please contact the Practice Secretary for further information. 

Complaints Procedure

We offer an informal in-house procedure to deal with any complaints about the services we provide.  Please ask our Practice Manager, Liz Cox, who will give you further information.  We attempt to resolve problems swiftly and take any appropriate action when we have failed to achieve the high standards we aim for.
